AHS Family Health Center was established in 2001 as an independent nonprofit organization with 501(c)(3) status. Our mission is to improve access to primary care for underserved communities on Chicago’s north side. In February 2002, we began operating a small two-room clinic, initially focused on providing Ob/Gyn and family practice-based care to immigrant and refugee families. Over time, AHSFHC has experienced significant growth and transitioned into a full-time Federally Qualified Health Center (FQHC) in the West Ridge neighborhood on March 1, 2004. We are proud to be recognized as a Patient-Centered Medical Home (PCMH).
At AHS Family Health Center, our primary commitment is to deliver a team-based approach, ensuring that our patients receive high-quality, comprehensive, coordinated, accessible, and, above all, patient-centered care based on evidence-based practices. By providing a comprehensive range of services, including primary, behavioral, and oral healthcare, we strive to be the go-to resource for all our patients’ healthcare needs. We believe in the power of prevention, which translates into better health outcomes and reduced medical costs for our patients and their families.
Our staff consists of highly trained medical and dental providers who are dedicated to serving our diverse patient population. All our providers are fluent in multiple languages, enabling our patients to communicate effectively in their preferred language. In instances where our providers or staff do not speak a particular language, we have access to interpreters who can assist our patients at no additional cost. At AHSFHC, we are committed to breaking down language barriers and ensuring that our patients receive the care they need in a culturally sensitive and linguistically appropriate manner.
